Vaccine Pass & Office Access

In line with the prevailing government policy, starting from 24 February 2022, except for those with a valid medical exemption certificate, anyone wishing to enter any part of HKCAAVQ’s premises will need to comply with the following:

 

 

  • Present evidence for at least the first dose of COVID-19 vaccination during the period from 24 Feb 2022 to end of April 2022; or

 

  • Provide a medical exemption certificate issued by a registered doctor or hospital as proof of having been assessed by a doctor as being medically unsuitable for vaccination. The validity period of the Exemption Certificate is pre-set at 90 days, with the longest being 180 days. The actual validity period will be decided by the doctor having regard to clinical conditions.

 

  • By late June, anyone above 18 years of age wishing to enter HKCAAVQ’s premises will need to have taken three doses of recognised COVID-19 vaccines, if it has been over nine months since their second dose. Those between 12 to 17 years of age must also have received their second dose if they have had their first dose for more than six months.

 

  • In view of the above, before entering the HKCAAVQ premises, all visitors are required to:

 

  • present vaccine records (or the medical certificate for vaccine exemption) under the Government’s “Vaccine Pass” requirements,
  • scan the “LeaveHomeSafe” venue QR code at the entrance, and
  • show HKCAAVQ’s reply email issued in response to your completion of HKCAAVQ’s electronic health declaration form.

 

  • The electronic health declaration form is available in the COVID-19 Control Button page of HKCAAVQ website. If the visitor did not complete the form prior visiting the HKCAAVQ premise, he/she will be required to complete the form at the reception using the iPad provided.
